Main Responsibilities:- Develop and implement personalized care plans, collaborating with healthcare providers and community resources.
- Assess member needs, identify gaps in care, and recommend interventions for improvement.
- Provide education and support to members and their families/caregivers on available services and benefits.
- Monitor care plans, member status, and outcomes, providing recommendations for improvement.
About You:
We are seeking a highly motivated individual with a Bachelor's degree and 2-4 years of related experience. Prior experience in social work, geriatrics, or human services is preferred. A RN or LCSW license is required.
