About the Role

This position involves conducting assessments of individual and carers' needs and risks to encourage independence, provide support, and prevent harm. As a qualified Social Worker, you will work in partnership with service users and carers using a person-centred approach, with the aim of maximising independence, wellbeing, and choice.

Key Responsibilities
  • Develop and implement care plans that promote independence and wellbeing.
  • Collaborate with multidisciplinary teams to provide holistic support to service users.

About the Benefits
