Job Title: Social Worker - Homeless Intervention

Are you a passionate and innovative social worker looking to make a difference in the lives of young people? Do you have experience working with homeless individuals and a desire to support them in achieving positive outcomes?

We are seeking a qualified social worker to join our Homeless Intervention Team, working with 16 and 17-year-old young people who are homeless or at risk of becoming homeless. As a social worker in this role, you will be responsible for assessing the needs of young people, developing and implementing support plans, and working with them to achieve their goals.

The Homeless Intervention Team is a dedicated team working with young people who are homeless or at risk of becoming homeless. We are committed to providing a supportive and non-judgmental environment, where young people can feel safe and empowered to make positive changes in their lives.

As a social worker in this role, you will have the opportunity to work with a diverse range of young people, each with their own unique experiences and challenges. You will be responsible for:

  • Assessing the needs of young people and developing support plans
  • Working with young people to achieve their goals and improve their well-being
  • Collaborating with other agencies and professionals to provide a comprehensive support package
  • Providing emotional support and guidance to young people
  • Monitoring and evaluating the effectiveness of support plans
