Job Purpose: To provide high-quality adult social care services to priority groups in a variety of settings.

Duties and Responsibilities:

  1. Deliver a wide range of adult social care services, including support for vulnerable adults, older people, and those with disabilities.
  2. Work independently, adhering to departmental and professional standards, to provide tailored care and support.
  3. Contribute to the development of departmental policies and procedures, ensuring effective working practices.
  4. Stay up-to-date with communication from the Trust and department, including emails and departmental updates.
  5. Complete necessary paperwork, including FP17 forms, to ensure accurate record-keeping.
  6. Manage patient charges and report any bad debtors to the Head of Adult Social Care.
  7. Provide adult social care services in the community, including domiciliary settings, to support housebound patients.
  8. Collaborate with healthcare professionals to provide adult social care services in hospital settings, under general anaesthesia, for patients who require it.
  9. Support the Head of Adult Social Care in maintaining health and safety standards, reporting risks, and taking appropriate action.
  10. Participate in audits and peer reviews to ensure high-quality care and support.
  11. Undertake any other duties and responsibilities within the framework of the post.

Key Responsibilities:

Information Governance: Ensure compliance with the UK GDPR and Data Protection Act 2018, protecting personal data and adhering to Trust policies.

Health & Safety: Contribute to a proactive risk management approach, assessing situations, taking appropriate actions, and reporting incidents and hazards.

Mental Capacity Act 2005: Stay up-to-date with the Act and deliver patient care in line with local and national policy and the MCA Code of Practice.

Safeguarding: Adhere to national and local safeguarding policies and procedures, acting upon concerns in accordance with them.

Infection Control: Adhere to Infection Prevention and Control Policies to ensure a clean environment and high standards of clinical care.

Smoke Free NHS: Comply with the Smoke-Free policy, not smoking on premises or surrounding land.

Training: Attend mandatory training identified for the role.

Equality and Diversity: Act in ways that support equality and diversity, working within the spirit and detail of legislation, including the Equality Act 2010 and Human Rights Act 1998.
