We are looking for an exceptional Paediatric Diabetes Service Coordinator to join our team. As a coordinator, you will play a vital role in supporting young people aged 12-19 living with diabetes, providing them with the necessary tools and resources to manage their condition effectively.

Your responsibilities will include developing and implementing support plans, conducting regular assessments to monitor progress, and collaborating with healthcare professionals to provide comprehensive care. You will also lead and deliver educational and recreational activities, promoting social, emotional, and educational skills in young people.

In addition to your clinical expertise, you will need to possess excellent communication and interpersonal skills, enabling you to work effectively with young people, families, and healthcare teams. A JNC Youth Work Qualification and experience working with young people in a healthcare or youth/community setting are essential requirements for this role.
