The salary for this position is £25,000 - £30,000 per annum, depending on experience.

Job Description:

  • Degree in a relevant field (e.g., social work, healthcare)
  • Minimum 1 year of experience in care coordination or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
