Health and Wellbeing Coordinator

We are seeking a highly motivated and compassionate individual to join our team as a Social Prescriber. As a key member of our Primary Care Network, you will play a vital role in supporting individuals, their families, and carers to take control of their well-being and improve their health outcomes.
Key Responsibilities- Provide personalized support to individuals, their families, and carers to address the wider determinants of health, such as debt, poor housing, physical inactivity, and low understanding of health conditions.
- Take a holistic approach to working with individuals, co-producing personal action plans to improve health and wellbeing, and reconnecting them with community resources.
- Work closely with individuals to identify and achieve their health and wellbeing goals, and provide ongoing support and guidance as needed.
- Collaborate with local charities and community organizations to ensure seamless referrals and support for individuals.
- Participate in regular supervision, appraisals, training, and other internal meetings to ensure effective communication and collaboration within the team.
- Knowledge of the local voluntary sector and local service provision, including asset-based community services.
- Understanding of person-centred approaches and the ability to work effectively with individuals from diverse backgrounds.
- Experience working with vulnerable adults, including those with mental health needs, and a strong understanding of safeguarding policies and procedures.
- Excellent communication and interpersonal skills, with the ability to work effectively with colleagues, patients, and external organizations.
- Ability to assess risks, anticipate difficulties, and successfully address them in a fast-paced and dynamic environment.
