We are looking for an experienced Practice Nurse to join our healthcare team at Kenmore Medical Centre in Wilmslow. As a skilled nurse, you will be responsible for providing high-quality patient care and managing long-term conditions.

Job Responsibilities:
- Assess, plan, develop, implement, and evaluate individual treatment plans for patients with known long-term conditions.
- Proactively identify and manage treatment plans for patients at risk of developing long-term conditions.
- Undertake agreed procedures, including blood pressure checks, pulse rate, and rhythm.
- Prioritise health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations.
