About The Role

We are seeking a compassionate and self-motivated individual to join our Reablement service as a Care Worker. In this role, you will support individuals who have been discharged from hospital or may have had an illness, crisis, fall at home or change in circumstances in relation to themselves or their careers.

Key Responsibilities

  • Support individuals to manage their own personal care within their own homes.
  • Focus on abilities and outcomes to assist individuals to improve their long-term independence.
  • Work alongside individuals to help them gain confidence and achieve goals.

  • No social care experience is necessary, we are more interested in your compassion and ability to care.
