Ackerman Pierce Ltd is seeking a dedicated Social Worker to join their Preparing for Adulthood team. The successful candidate will work closely with families and service users, promoting their views and wellbeing.

Key Responsibilities:
  • Progress cases from Children Services to Adult Services
  • Collaborate with Special Education Needs Coordinators on funding matters
  • Work jointly with Health services within the integrated Community Team
  • Attend and organize relevant meetings
  • Raise and manage safeguards on allocated cases
  • Present cases at the Risk Panel when required
Requirements:
  • HCPC Registration
  • 1-2 years post-qualification experience in UK Social Care
  • Enhanced DBS/CRB check
  • Eligibility to work in the UK
