Their facilities cater to young people aged 12-17 and include secure training centres with an 80-bed capacity.
The organisation delivers NHS-standardised healthcare services, including GP, dental, optician, physiotherapy, and podiatry services. They also provide immunisations, sexual health clinics, and specialised programs such as Young Fatherhood initiatives.
The team comprises registered nurses across various disciplines, including learning disability, mental health, and children's nursing.
Key Responsibilities- Manage your own caseload of patients with diagnosed Learning Disabilities or Autism Spectrum Disorders (ASD) and develop appropriate care plans.
- Conduct initial assessments for new referrals and ensure follow-up procedures are in place.
- Participate in processes designed to assist at-risk patients, adhering to NICE Guidance.
- Employ excellent communication and interpersonal skills to support patients and staff.
- Provide awareness training and advice on patient care to colleagues.
- Collaborate with the Primary Care Mental Health Team to deliver awareness days and workshops.
