We are seeking a compassionate and dedicated Community Nurse to join our team in providing high-quality care and support to patients in their homes. As a Primary Care Practitioner, you will play a vital role in delivering patient-centered care and improving health outcomes in the Kent community.
Key RequirementsTo be successful in this role, you will need:
- Valid Nursing and Midwifery Council (NMC) registration
- Proven experience as a Community Nurse or similar role
- Ability to work autonomously and within a multidisciplinary team
- Excellent communication and interpersonal skills
- A valid driving license and access to a car
