**Job Summary**

Adult Social Care is seeking a highly skilled and compassionate Child and Family Care Specialist to join our team. As a Family Practitioner, you will play a vital role in supporting disabled children and their families in partnership with education and health professionals.

Main Responsibilities

  1. Work closely with disabled children and their families to identify and address their needs, ensuring they receive the necessary support to achieve their full potential.
  2. Manage a caseload of children and families, providing guidance and support to social work professionals and other stakeholders.
  3. Deliver Family Help services, working autonomously to support families in need.
  4. Conduct thorough assessments of family needs, identifying resources and services that can help improve their situations.
  5. Collaborate with education and health professionals to ensure disabled children are included and supported in their educational and healthcare journeys.

Requirements

  • Proven experience working with disabled children and their families, with a strong understanding of their needs and challenges.
  • Full UK driving license and access to a vehicle, with a commitment to traveling to meet the needs of service users.
  • Enhanced DBS clearance, with a commitment to maintaining the highest standards of professionalism and confidentiality.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with families, professionals, and other stakeholders.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
