About the Role

We are seeking a highly motivated and compassionate Rehabilitation Assistant to join our team at Central London Community Health Trust. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and supporting our patients to achieve their rehabilitation goals.

Key Responsibilities
  • Contribute to the initial and ongoing assessment of patients by completing relevant paperwork and recording and reporting feedback to the therapist.
  • Prepare for and assist with clinical activities delivered by the therapist.
  • Adhere to universal precautions, including hand washing and disposal of waste, and ensure effective cleaning of equipment before and after the activity.
  • Undertake prescribed therapy activities and ADL assessments that you have previously been signed competent for, and report any variances or concerns back to relevant team members.
  • Assess safety of conditions and environment prior to in/outdoor rehabilitation activities.
  • Provide clear communication and explanations to patients and effectively respond to questions posed during interventions with no direct supervision.
  • Coach other support staff in undertaking prescribed therapeutic interventions, and in the use of specific equipment.
  • Enable clients to maintain and improve their independence by fitting prescribed equipment, demonstrate and educate safe use of equipment.
Requirements
  • Educated to GCSE level, with English and Mathematics at grade C or above.
  • NVQ level 3 in Health and Social Care or Promoting Independence or Rehabilitation or equivalent undergraduate award.
  • Moderate experience of working in a caring role in the NHS for at least a year, including patients with physical disabilities.
  • Experience working as a member of a team.
  • Sound understanding of confidentiality.
  • Knowledge of recent NHS developments and how they may impact on patient care.
  • Current issues in the delivery of Services for older people.
