Job Title: Supported Housing Case Worker

We are seeking a highly motivated and compassionate Supported Housing Case Worker to join our team in the Brynmawr Area. As a key member of our team, you will play a vital role in supporting individuals with mental health problems to achieve their goals and live independently.

About the Role:
  • Provide front-line support to service users, building trusting relationships and empowering them to make informed decisions about their lives.
  • Work closely with service users to develop and implement personalized support plans, ensuring their needs are met and their goals are achieved.
  • Collaborate with other professionals, including healthcare providers and social services, to ensure a comprehensive and coordinated approach to support.
  • Conduct regular reviews and assessments to monitor progress and make adjustments to support plans as needed.
  • Provide emotional support and guidance to service users, helping them to manage their mental health and well-being.
Requirements:
  • Experience working with individuals with mental health problems and a strong understanding of the challenges they face.
  • Excellent communication and interpersonal skills, with the ability to build rapport with service users and other professionals.
  • Ability to work independently and as part of a team, with a flexible and adaptable approach to meet the needs of service users.
  • High level of emotional intelligence, with a non-judgmental and empathetic approach to supporting individuals.
  • Valid driving license and ability to travel as part of the role.
