Developmental Learning Disability Care Coordinator

We are seeking an experienced Learning Disability Nurse to join our team in North Wales. As a key member of our multidisciplinary team, you will have the opportunity to make a real difference in the lives of individuals with learning disabilities and complex health needs.
Responsibilities- Conduct thorough assessments to identify individual needs and develop targeted interventions.
- Work collaboratively with other healthcare professionals to plan and deliver effective care strategies.
The successful candidate will have a salary range of approximately £45,000-£65,000 per annum, depending on experience and qualifications.
