Job Overview

We are seeking a highly skilled and compassionate Community Support Specialist to join Corus Consultancy in Bexleyheath. As a valued member of our team, you will be responsible for providing exceptional care and support to older people, including those with mental health needs, and individuals with physical disabilities, within their own homes or community settings.

About the Role

This is an exciting opportunity to work collaboratively with multi-disciplinary teams to deliver timely and effective care plans that maximize independence and promote a 'own bed is best' ethos.

Key Responsibilities
  1. Conduct thorough assessments of social care/rehabilitation needs, identifying solutions that meet the individual's requirements and facilitate preventative/rehabilitative care.
  2. Manage a caseload, working closely with colleagues to deliver high-quality social care services that cater to diverse needs.
  3. Develop and implement individual care plans in consultation with clients, carers, and professional colleagues, providing ongoing support and guidance as needed.
  4. Refer service users to relevant professionals, such as social workers, care managers, occupational therapists, and nursing staff, when specific needs arise during assessment, review, or supervision.
  5. Assist and support colleagues by undertaking tasks as requested, which may include arranging transportation and escorting service users as necessary.
Requirements
  • Demonstrable experience working with older adults, individuals with disabilities, and carers, preferably in a health or social care setting.
  • In-depth understanding of the needs of older people, individuals with disabilities, and/or those with mental health issues.
  • Ability to work independently in a community setting, respecting clients' privacy and confidentiality while maintaining excellent communication skills.
  • Commitment to safeguarding and protecting vulnerable service users.
  • Effective verbal and written communication skills, with the ability to articulate complex ideas clearly and concisely.
Salary and Benefits

The salary for this role is £28,000 - £32,000 per annum, commensurate with experience. We offer a comprehensive benefits package, including opportunities for career development and growth within our organization.
