Job Summary

As an End-of-Life Care Specialist, you will work closely with our multidisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.

Key Responsibilities

  • Deliver high-quality care to patients with life-limiting illnesses, including those with cancer, heart disease, and other conditions.
  • Provide emotional support and companionship to patients and their families, helping them cope with the physical and emotional challenges of end-of-life care.
  • Work collaboratively with healthcare professionals to develop and implement comprehensive care plans that address the physical, emotional, and spiritual needs of patients.
  • Maintain accurate and up-to-date records of patient care, including progress notes, medication administration records, and other relevant documentation.
  • Participate in ongoing education and training to stay current on best practices in end-of-life care and palliative medicine.

Requirements

To be successful as an End-of-Life Care Specialist, you will possess:

  • A National Vocational Qualification (NVQ) Level 2 in Health and Social Care or equivalent qualification.
  • Functional skills at level 2 in numeracy and literacy or equivalent.
  • 6 months' experience in a health or social care environment, preferably in a palliative care setting.
  • The ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • A strong commitment to delivering exceptional patient care and a passion for working with individuals with life-limiting illnesses.
