We are seeking a highly skilled and experienced Senior Primary Care Nurse to join our Primary Care team at Practice Plus Group, a leading healthcare provider. As a Senior Primary Care Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our team.

Key Responsibilities

  • Provide expert primary care services to patients, including health assessments, treatment, and management of chronic conditions
  • Lead and manage a team of primary care nurses, providing guidance and support to ensure excellent patient care
  • Develop and implement effective care plans, working closely with other healthcare professionals to ensure seamless patient care
  • Contribute to the development and implementation of primary care services, ensuring they meet the needs of our patients and stakeholders

Requirements

  • Current NMC registration as a nurse
  • Proven experience in primary care, with a strong understanding of healthcare policies and procedures
  • Excellent leadership and management skills, with the ability to motivate and inspire a team
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, colleagues, and stakeholders
