About the Role

Epsom and St Helier University Hospitals NHS Trust is seeking an enthusiastic Occupational Therapist to join our Sutton Health and Care at Home service.

As a key member of our multidisciplinary team, you will play a vital role in preventing admissions from the community, accelerating discharge from emergency departments and acute medical wards, and providing supported discharge through bed and home-based intermediate care.

Key Responsibilities

  • Work collaboratively with our multidisciplinary team to provide comprehensive occupational therapy services to patients in their homes.
  • Conduct thorough assessments and develop personalized care plans to meet the unique needs of each patient.
  • Provide training and education to patients, families, and carers to promote independence and self-management.
  • Work closely with our community teams to ensure seamless transitions and effective communication.

Requirements

  • Degree in Occupational Therapy or equivalent qualification.
  • Registration with the Health and Care Professions Council (HCPC).
  • Experience of working in a community setting, preferably with older adults or individuals with complex needs.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
