We are currently seeking a highly skilled Community Care Coordinator to join our Community Mental Health Team based in Liskeard. As a key member of our team, you will be responsible for assessing care needs and providing clinical management to patients with severe and enduring mental health problems.

The ideal candidate will have a core profession in mental health nursing and experience working independently in a mental health setting. You will also possess excellent communication skills, with the ability to work effectively with patients, carers, and other healthcare professionals.

About the Role

  1. To assess and manage complex mental health cases, working closely with multidisciplinary teams to ensure comprehensive care plans.
  2. To develop and implement innovative solutions to improve patient outcomes and experiences.
  3. To facilitate effective communication and collaboration between primary and secondary services, ensuring seamless transitions for patients and carers.

You will need to have knowledge of Dialog+, legislation around mental health, and The Mental Health Act.
