About the Role

Oxleas NHS Foundation Trust is seeking a skilled Mental Health Practitioner to join our team at HMP Guys Marsh. As a key member of our Mental Health In-Reach Team, you will provide specialist mental healthcare to offenders, working in a psychologically minded way to achieve their agreed goals and quality health outcomes.

Key Responsibilities

  • Provide comprehensive assessments, screenings, and interventions to offenders with complex mental health needs.
  • Develop and implement care plans, working collaboratively with other healthcare professionals and statutory agencies.
  • Contribute to the development and implementation of high-intensity interventions and complex case management arrangements.
  • Work closely with community mental health teams to ensure continuity of care and the Care Programme Approach for all offenders.

Requirements

  • Professional registration as a Mental Health Professional (e.g., RMN, HCPC, Social Worker).
  • Experience of working with offenders and complex mental health needs.
  • Excellent communication and interpersonal skills.
