Job Description

We are seeking a highly skilled Prescribing Pharmacist to join our team, working 37.5 hours per week. As a key member of our multidisciplinary team, you will be responsible for prescribing, online and telephone clinical triage, management of minor illness and LTC advice. You will deliver safe, clinical decisions and expert care, working collaboratively with all team members to drive innovation, modernisation, and high levels of staff and patient satisfaction.

Key Responsibilities
  1. Prescribing medication and making informed decisions about patient care.
  2. Providing online and telephone clinical triage to assess patient needs and prioritize care.
  3. Managing minor illnesses and providing Long-Term Condition (LTC) advice to patients.
