Specialist Practitioner District Nurse & Community Nurse Practitioner

We are seeking highly skilled and dedicated individuals to fill the role of Specialist Practitioner District Nurses and Community Nurse Practitioners across Edinburgh and East Lothian.

Key Responsibilities:
  • Provide high-quality nursing care to patients in their homes and community settings.
  • Work collaboratively with multidisciplinary teams to deliver patient-centered care.
  • Develop and implement individualized care plans to meet the unique needs of patients.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
  • Collaborate with patients, families, and caregivers to ensure seamless care transitions and optimal patient outcomes.
Requirements:
  • Registered Nurse with current registration and a valid practicing certificate.
  • Postgraduate qualification in District Nursing or a related field.
  • Proven experience in community nursing, preferably in a District Nursing role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse patient populations.
  • Strong assessment and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
