Job Title: Clinical Nurse Specialist / AHP for Virtual Wards - North Cumbria

Job Summary: We are seeking a highly skilled Clinical Nurse Specialist / AHP to join our Virtual Ward team in North Cumbria. The successful candidate will play a key role in coordinating the hospital-based and community-based elements of the ward, promoting the benefits of the virtual ward to patients and healthcare professionals, and delivering specialist expertise to ensure patients feel supported on the virtual ward.

Main Responsibilities:
  • Coordinate the hospital-based and community-based elements of the virtual ward
  • Promote the benefits of the virtual ward to patients and healthcare professionals
  • Assess and remote monitor patients in the community
  • Deliver specialist expertise to ensure patients feel supported on the virtual ward
  • Employ holistic assessment and strategies to manage emerging health needs
  • Provide administrative duties to facilitate the smooth operation of the service
  • Integrate into the wider Respiratory directorate
Requirements:
  • Be an integral member of the virtual ward team with partnership working across the virtual ward MDT
  • Develop specialist knowledge, skills, and experience across an array of medical conditions suitable for virtual ward management
  • Provide specialist advice, guidance, and care to patients and their carers
  • Hold delegated responsibility for the virtual ward patient caseload, coordinating admissions and discharges for patients referred to the service using digital technology
  • Assess and treat patients requiring acute level care, including the provision of acute therapy if deemed appropriate within the virtual ward setting
  • Ensure compliance with Professional body regulations and with Trust policies, procedures, and protocols
  • Maintain knowledge, skills, abilities, and attitudes necessary to deliver an evidence-based service that is responsive to the changing needs of the locality population and the service
  • Provide and participate in clinical supervision to ensure high clinical standards and facilitate the development of individual team members
  • Support other members of the virtual ward as required and deputise, where appropriate
