Key Responsibilities- Conduct comprehensive assessments of care needs and develop individualized care plans to meet the needs of service users.
- Work collaboratively with other healthcare professionals to provide holistic care and support to service users.
- Participate in team meetings and contribute to the development of new methods and models of care.
- Ensure effective communication between all care professionals and provide significant data governing client care and treatment.
- Develop close links with relatives and carers to ensure effective communication and follow-up care.
- Advise the manager and Professional Nursing Advisor on breaches of nursing professional conduct.
- Liaise with the Manager to ensure that all Trust policies and procedures are communicated, implemented, and developed within the area.
- Participate in professional supervision on all allocated cases.
