About the Role
The Suttons Wharf Health Centre is seeking a highly skilled and organised individual to fill the position of Care Coordinator. This role is a critical part of our team, responsible for ensuring that patients receive the right care at the right time.
Main Responsibilities
- Act as a primary point of contact for patients, providing comprehensive information and support throughout their care journey.
- Coordinate patient appointments, referrals, and follow-ups, ensuring seamless communication between different services and departments.
- Generate and manage patient searches and lists in EMIS Web, tailoring these searches and reports to various KPIs and understanding how to manipulate and interpret them.
- Collaborate with the healthcare team to develop and manage personalised care plans for patients, particularly those with complex needs or long-term conditions.
- Organise and attend Multi-disciplinary Team (MDT) meetings as needed, ensuring all relevant information is available and actions are documented and followed up.
- Provide comprehensive administrative support to the clinical team, including maintaining patient records, updating care plans, and managing correspondence.
- Advocate for patients, ensuring their needs and preferences are understood and respected, and facilitate communication between patients, families, and healthcare providers.
- Maintain accurate and up-to-date patient information in the practice's electronic health records system, generating reports and analysing data to support quality improvement initiatives.
