About the Role

We are looking for an experienced Community Care Coordinator to join our team in North and East Wiltshire. As a Band 6 Specialist Recovery Practitioner, you will play a vital role in coordinating care for service users with complex mental health needs.

The successful candidate will have a strong background in mental health nursing or a related field and be able to work effectively with individuals, families, and carers to develop personalized care plans.

You will be working closely with other healthcare professionals to provide comprehensive assessment, treatment, and support to service users with psychotic illnesses and co-existing substance misuse issues.

Salary: £38,052 - £44,688 per annum (pro rata for part-time)

Key Responsibilities
  1. Conduct thorough assessments of service users' mental health needs and develop tailored care plans.
  2. Collaborate with multidisciplinary teams to coordinate care planning, risk management, and crisis interventions.
  3. Foster strong relationships with service users, carers, and healthcare professionals to ensure effective care coordination.
Requirements
  • Degree-level qualification in a relevant health profession.
  • Current registration with the NMC or HCPC.
  • UK driving licence with business insurance.
