Key Responsibilities

  • Provide high-quality care and support to individuals, ensuring their physical, emotional, and social needs are met.
  • Develop and implement personalized care plans, working closely with the individual, their family, and other healthcare professionals.
  • Lead and supervise a team of Support Workers, promoting a culture of excellence and continuous improvement.
  • Collaborate with other healthcare professionals to ensure seamless care and support.
  • Participate in social activities, outings, and holidays with individuals, promoting their engagement and enjoyment.
  • Maintain accurate records and reports, adhering to regulatory requirements and organizational standards.

Requirements

  • NVQ 3 or equivalent in care.
  • Minimum of two years' experience working with vulnerable adults.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of key worker systems and personal care planning.
