Key Responsibilities
- Provide high-quality care and support to individuals, ensuring their physical, emotional, and social needs are met.
- Develop and implement personalized care plans, working closely with the individual, their family, and other healthcare professionals.
- Lead and supervise a team of Support Workers, promoting a culture of excellence and continuous improvement.
- Collaborate with other healthcare professionals to ensure seamless care and support.
- Participate in social activities, outings, and holidays with individuals, promoting their engagement and enjoyment.
- Maintain accurate records and reports, adhering to regulatory requirements and organizational standards.
Requirements
- NVQ 3 or equivalent in care.
- Minimum of two years' experience working with vulnerable adults.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of key worker systems and personal care planning.
