Responsibilities:

  • Provide Case Management Services: Under the direction of the Manager or Clinical Coordinator, the Case Manager will provide case management to members with high-risk or complex medical and psycho-social problems through community and home-based visits and telephonic support.
  • Link Members to Care Services: The Case Manager will provide linkage to appropriate community resources, referral to appropriate care services, and communicate with providers to reduce health disparities and improve healthcare outcomes.
  • Act as an Advocate: The Case Manager will act as an advocate for members to assure access and service delivery across the continuum of care and community resources.
  • Develop Targeted Initiatives: The Case Manager will develop and implement targeted initiatives based on knowledge and expertise in clinical practice and disease states that promote improved healthcare outcomes and quality of life.

Requirements:

  • A current NYS Professional Nursing License is required.
  • A minimum of 2 years' experience in medical case management and 2 years' recent acute care and/or community health nursing with knowledge of community resources, healthcare services, treatment planning, and discharge planning.
  • Strong computer skills.
  • Ability to read, understand, and follow written policies and procedures.
  • Ability to interact with individuals with diverse cultural and religious customs.
  • Must have a NYS driver's license and car for community travel.

Compensation:

The estimated annual salary for this position is $65,000 - $80,000, depending on experience.
