About the RoleThis is an exciting opportunity to work in a dynamic and innovative community service, providing rapid response to patients in their own homes who are at risk of hospital admission. As a Senior Occupational Therapist, you will be part of a friendly and approachable team who work well together, and will be well supported by your senior management team.
Main Responsibilities- Provide occupational therapy services to patients in their own homes, focusing on maintaining independence and avoiding hospital admission.
- Work closely with other healthcare professionals to prioritize caseloads and provide responsive care to patients.
- Develop and implement individualized care plans to meet the needs of patients with a wide range of conditions, including neurological, orthopaedic, falls, long-term conditions, and musculoskeletal.
- Collaborate with other community services, including health and social care, to ensure seamless care transitions.
- Contribute to the development and implementation of occupational therapy services, including policy development and quality improvement initiatives.
- Experience of working at B5/B6 level in a community or hospital setting.
- Previous NHS experience, with a strong understanding of the NHS values and principles.
- Degree in Occupational Therapy and HCPC registration.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work flexibly, including weekends and bank holidays, with a willingness to participate in on-call rotas.
