Job Summary

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Sussex Community. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality patient care and contributing to the development of our services.

Main Responsibilities
  • Conduct holistic assessments and develop individualized care plans to promote patient independence and well-being.
  • Collaborate with external partners and patients' families to ensure seamless care transitions and effective communication.
  • Participate in service provision and change initiatives to drive continuous improvement and excellence in patient care.
  • Engage in professional development to maintain area of practice and stay up-to-date with current local policies and research-based practices.
Requirements
  • NMC Registration
  • 1 year post-registration experience in a hospital setting
  • Ability to demonstrate application of research-based practice and knowledge of current local policies
  • Basic computer skills or willingness to achieve within an agreed period
What We Offer

We offer a competitive salary of £29,970 to £36,483 per annum, plus a range of benefits, including free on-site parking and opportunities for professional development.
