Derbyshire County Council is looking for an experienced and passionate Transition to Adulthood Specialist to join their Transformation and Partnership Team.

The ideal candidate will have knowledge of the Care Act 2014 and The Health and Care Act 2022, as well as experience working with young people preparing for adulthood.

The successful candidate will be responsible for completing strengths-based timely assessments, reviews, support planning, and working in collaboration with colleagues to achieve positive and more independent outcomes for individuals.

This is a full-time post for a fixed term of up to 2 years or internal secondment.

The Transition to Adulthood team looks at using innovative forward-thinking approaches to meet the assessed eligible social care needs of young people.

The successful candidate will have good communication skills and an ability to travel is essential due to the county-wide nature of the role.
