Job Summary

Key Responsibilities
  • Conduct comprehensive assessments of individuals' needs, including physical, emotional, and social factors.
  • Develop and implement person-centered care plans in collaboration with clients and their families.
  • Advocate for clients' rights and ensure their voices are heard in the decision-making process.
  • Provide support and guidance to clients facing complex challenges, including mental health issues, disabilities, and social isolation.
  • Collaborate with multi-disciplinary teams, including health professionals, to ensure integrated service delivery.
  • Monitor and review care plans regularly, making adjustments as necessary to meet changing needs.
  • Maintain accurate and up-to-date case records in accordance with legal and organisational standards.

Qualifications
  • A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
