Main Purpose of Post:

To provide comprehensive support and care to individuals with mental health issues, ensuring their well-being and promoting social inclusion.

Liaise with professionals to ensure seamless care pathways, safeguarding and promoting independence.

Promote joint working to deliver person-centered outcomes for individuals with mental health issues.

Undertake safeguarding investigations as required.

Assist the Local Authority in meeting statutory obligations under the Mental Health Measure and the Social Services and Wellbeing (Wales) Act 2014.

Key Responsibilities:
  1. Assess and manage the complex needs of individuals with mental health issues and their carers.
  2. Maintain a caseload and ensure timely administration of all aspects.
  3. Assess risks to individuals with mental health issues and others, promoting independence and safeguarding.
  4. Make recommendations on allocating scarce resources.
  5. Ensure equal opportunity and respect for diversity and difference.
  6. Promote social inclusion of isolated and vulnerable individuals.
  7. Investigate abuse of vulnerable adults.
  8. Work collaboratively with other agencies.
  9. Accountable for actions through effective recording of decisions.
  10. Safeguard the welfare of vulnerable individuals at immediate risk of harm.
  11. Engage in direct work with individuals with mental health issues and carers.
  12. Enhance problem-solving capacities to support maximum independence and choice.
  13. Consider options to best meet assessed eligible needs and assist in making informed decisions.
  14. Undertake the role of an AMHP.
  15. Conduct Mental Capacity Act assessments and Best Interests decisions.
  16. Adopt a creative and holistic approach.
  17. Maximize financial and material resources available to service users.
  18. Record unmet need.
  19. Collate assessment findings and complete documentation.
  20. Prepare court and specialist reports.
  21. Ensure case file recording and documentation is completed.
  22. Manage conflict effectively.
  23. Use professional assertiveness to justify decisions.
  24. Challenge judgements and decisions of others.
  25. Convene and attend interagency meetings.
  26. Obtain managerial agreement for personal plan of care delivery.
  27. Record and disseminate care and treatment plans.
  28. Contribute to shaping services.
  29. Understand budget and resource implications.
  30. Encourage positive risk-taking.
  31. Delegate monitoring tasks.
  32. Arrange and coordinate review of Care and Treatment Plan.
  33. Engage with individuals, families, and carers in review.
  34. Negotiate and agree changes to plan.
  35. Ensure individuals are aware of complaints procedures and advocacy services.
  36. Contribute to professional development of others.
  37. Prepare for and engage in practice observations.
  38. Contribute to team and peer learning programmes.
  39. Strive to be an effective team player.

Salary Expectations:

Regular Pay: £33.60/Hour

Bank Holiday Rate: £67.20/Hour

Overtime Rate: £44.69/Hour
