Home-Based Clinical Support Professional

We are seeking a Home-Based Clinical Support Professional to join our team in Edinburgh and East Lothian. As a key member of our Care at Home team, you will provide vital support to patients and their families, promoting independence and improving quality of life.
You will work closely with registered nursing staff and other healthcare professionals to develop and implement personalized care plans, ensuring seamless coordination of care across settings.
In this role, you will have the opportunity to make a real difference in the lives of patients and their families, while developing your skills and expertise in palliative care.
Responsibilities:- Providing comprehensive support to patients and their families, addressing physical, emotional, and social needs
- Developing and implementing individualized care plans in collaboration with multidisciplinary teams
- Facilitating communication between healthcare providers, patients, and families to ensure coordinated care
- Participating in regular team meetings and contributing to service development and improvement initiatives
We offer a competitive salary of £27,933 per annum (rising to £29,050 subject to satisfactory performance review), plus benefits, including annual leave allowance, occupational sick pay entitlements, and access to ongoing education and training opportunities.
