Crisis Support Specialist - Tripod Partners

About the Role

We are seeking a highly motivated Crisis Support Specialist to join our team at Tripod Partners. As a key member of our adult care services, you will play a vital role in ensuring the safety and wellbeing of vulnerable adults.

  • Rapidly assess referrals sent to our services to identify potential risks and needs.
  • Develop and maintain relationships with professional services, including CMHT, OPCMHT, Police, Hospital, and GP's, to ensure effective collaboration and support.
  • Locate relevant community resources, such as Food Banks, BAME Groups, Men/Women/LGBT+ Support Groups, to provide individuals with necessary support.
  • Conduct initial Needs Assessments to determine appropriate housing solutions and support requirements.
  • Carry out home visits where required to assess individual circumstances and provide guidance.
  • Recognize and respond to identified risks, taking prompt action to ensure the safety and wellbeing of service users.
  • Support Enquiry Officers in managing calls received that are concerns, referring them to CAD Triage for further assistance.
  • Manage EDT Notifications by carrying out further enquiries or signposting to relevant teams.
  • Review SF1 forms and return them to CAD Triage if they do not meet SG Team criteria.
About You

To be successful in this role, you will need to be a qualified Social Worker with experience in adult social care. You must be passionate about making a positive difference in the lives of vulnerable adults.
