Job Description:
  1. Provide comprehensive assessments and reports as part of the allocated Care Coordinator/adults social care social worker team for young people.
  2. Work collaboratively with professionals to deliver Pathway Planning, assessments, tasks, and interventions that strengthen mental health and wellbeing of Care Experienced young people within the Borough.
  3. Serve as a Lead Practitioner within the team for Mental Health, sharing best practices, presenting at Team Meetings, and acting as a Mental Health practice reference point for colleagues.
  4. Establish and develop partnerships with allied health and social care practitioners from adults services, as well as partners in Housing, DWP, and Community organisations.
  5. Demonstrate expertise in evidence-based practice and integrate models of best practice into day-to-day service delivery.
  6. Participate positively in professional supervision and annual appraisal (PrP) review, taking personal responsibility for quality of practice.
  7. Participate in clinical supervision and risk assessing, and liaise with mental health services across different boroughs/areas.
  8. Build professional credibility within the multi-professional partnership by achieving practice standards and meeting performance targets consistently; li>
  9. Actively contribute to team working, supporting colleagues, covering for colleagues in their absence, and participating in team meetings and development events.
  10. Consider and propose ideas for improving efficiency and effectiveness of Council Services to children and families.
  11. To understand both Council's duties and responsibilities for safeguarding children, young people and adults as they apply to the role within the council.
