About the Role

We are seeking an experienced Palliative Care Specialist Nurse to join our team at the Northern Care Alliance NHS Foundation Trust. This is a challenging yet rewarding opportunity for a dedicated professional to make a real difference in the lives of patients and their families.

Job Summary

The successful candidate will be responsible for providing high-quality palliative care to patients with life-limiting illnesses, working closely with multidisciplinary teams to coordinate care and ensure seamless transitions. They will also provide education and training to healthcare professionals on palliative care principles and practices.

Key Responsibilities
  • Provide specialist palliative care to patients with cancer or other life-limiting illnesses
  • Coordinate care with nursing, medical, and AHP colleagues across departmental and organisational boundaries
  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Collaborate with families and carers to ensure they receive the support they need
Requirements

To be successful in this role, you will need:

  • A current NMC PIN as a Registered Nurse or Midwife in the UK
  • Post-registration qualification in palliative care or relevant courses/study days
  • Previous Band 6 experience in a similar role
  • Ability to travel to different geographical locations within the Trust footprint
