About the Role:

We are seeking a highly skilled and compassionate Nurse Case Manager to join our team in the North West. As a Community Nurse Case Manager, you will play a vital role in supporting clients to live independently at home and within their communities.

Key Responsibilities:

  • Assess and develop care plans to meet the individual needs of clients
  • Provide expert nursing care and support to clients in their own homes
  • Work collaboratively with multidisciplinary teams to ensure seamless care delivery
  • Conduct regular reviews and updates to care plans as needed

Requirements:

  • Registered General Nurse (RGN) or Registered Sick Children's Nurse (RSCN) qualification
  • Experience in community nursing or a related field
  • Strong communication and interpersonal skills
  • Ability to work autonomously and as part of a team
