Job Description:
We are seeking a dedicated and experienced Registered Case Manager to join our team at Leaders in Care. As a Registered Case Manager, you will be responsible for overseeing complex care packages for individuals with learning disabilities and autism.
Key Responsibilities:
- Oversee all aspects of running complex care packages, ensuring the highest standards of care and support.
- Ensure compliance with the Health and Social Care Act 2008 and associated standards.
- Travel between care packages from various locations, with a focus on ensuring seamless care and support.
- Attend regular meetings with the wider team to ensure alignment and collaboration.
- Lead by example and cover shift shortfalls when required.
- Participate in local authority provider and engagement forums.
- Liaise with commissioners, safeguarding boards, and specialist professionals.
Requirements:
- Hold or be willing to work towards an NVQ Level 5 in Health and Social Care or equivalent.
- Experience in roles such as Care Manager, Case Manager, Health and Social Care Manager, Support Services Manager, or Complex Care Manager.
- Be approachable and supportive, fostering open communication.
- Be adaptable and flexible, willing to cover shifts and travel as needed.
What We Offer:
- Annual salary of £40,000 - £45,000 (depending on experience).
- £3,000 car allowance.
- 25 days annual leave plus statutory bank holidays.
