Nurse Specialist in Chronic Disease Management

We are seeking a skilled and dedicated Nurse Specialist in Chronic Disease Management to join our team at Beacon Medical Group. As a key member of our healthcare team, you will be responsible for providing high-quality care to patients with chronic diseases, including diabetes, asthma, and COPD.
About the RoleThis is a full-time position that requires excellent communication and interpersonal skills, as well as a strong understanding of chronic disease management principles. The successful candidate will have experience working with patients with chronic conditions and will be able to provide comprehensive care, including diagnosis, monitoring, and development of individual management plans.
Responsibilities- Manage chronic disease patients, including those with diabetes, asthma, and COPD.
- Develop and implement individualized care plans for patients with chronic conditions.
- Monitor patient progress and adjust treatment plans as needed.
- Collaborate with other healthcare professionals to ensure comprehensive care for patients.
- Provide education and support to patients and their families on managing chronic conditions.
To be successful in this role, you will need:
- A registered general nursing qualification (RGN) or equivalent.
- Experience working with patients with chronic conditions.
- Strong communication and interpersonal skills.
- Able to work independently and as part of a team.
- Ability to prioritize tasks and manage time effectively.
- Familiarity with electronic health records systems.
- DBS clearance (successful applicant will be required to complete this).
We offer a competitive salary package, including a base salary of £45,000 - £60,000 per annum, depending on experience, plus benefits such as:
