About the Role:

Care Team Lead:

We are seeking a dedicated and compassionate individual to join our team as a Care Team Lead. As a key member of our care team, you will be responsible for planning and implementing high-quality care services for residents in our care homes. Your primary goal will be to ensure that every resident receives personalized care and support tailored to their unique needs.

As a Care Team Lead, you will work closely with our Registered Nurses and Care Managers to develop and implement comprehensive care plans. You will also provide guidance and support to our care staff to ensure they have the necessary skills and knowledge to deliver exceptional care.

Key responsibilities include assisting in the management of care teams, conducting regular assessments, and maintaining accurate records of resident care. A strong background in person-centered care planning and knowledge of caring for people with dementia are essential requirements for this role.
