{"Job Title": "Medical Coder", "Job Summary": "We are seeking a skilled Medical Coder to join our Coding/Summarising Team at The Lodge Health Partnership. As a Medical Coder, you will be responsible for ensuring that patient records are up-to-date and accurately coded and recorded on our Practice clinical system, EMIS Web. This will involve receiving patient information and communication, scanning and inputting data, and prioritizing and distributing patient information and correspondence. You will also be responsible for ensuring that S17/S47 safeguarding forms are completed, filed, and sent to the appropriate team in a timely manner.

Key Responsibilities:

* Receive patient information and communication via various methods
* Scan, input, and accurately code daily communications to record medical information in patient records
* Prioritize and distribute patient information and correspondence
* Ensure S17/S47 safeguarding forms are completed, filed, and sent to the appropriate team
* Ensure safeguarding issues are raised with the Practice Safeguarding Lead or Practice Manager
* Produce and maintain an accurate coded medical summary
* Ensure all essential history held in paper records is recorded in electronic records
* Accurately code and prioritize information to aid patient consultations, referrals, and prescribing
* Practice audit and targets
* Patient recall systems for screening, immunizations, chronic disease management, or other relevant purposes
* File and retrieve information as required
* Ensure all QOF related data is captured
* Examine records and documentation for missing information
* Manage own workload to ensure work is accurate and complete
* Resolve coding queries by consulting with line manager, colleagues, and medical staff
* Undertake system searches to improve data quality and coding accuracy for disease registers

Person Specification:

* Good sound secondary education
* Medical experience
* Medical information or medical secretarial experience
* Good English language and writing ability
* Computer-literate with keyboard skills
* Basic understanding and ability to recognize significant medical history and problems
* Efficient, organized, and methodical
* Supportive team worker
* Able to work by self and use own initiative
* Discreet and confidential
* Attention to detail
* Able to work under pressure and meet deadlines
* Keen learner and able to support others
* Professional and friendly person

Desirable Qualifications:

* Medical qualification
* Medical coding qualification
* IT qualification
* Worked in General Practice or NHS Trust information team
* Computerized record-keeping
* GP information searches, audit, and/or supporting patient service recall systems
* emisWeb or other GP clinical system
* Read and/or Snomed coding
* Open Exeter IT system
