Home Care Operations Coordinator

Brook Street is seeking a highly organized and detail-oriented individual to fill the role of Care Coordinator. As a Care Coordinator, you will play a critical part in ensuring that our clients receive the highest quality care services. Your primary responsibilities will include scheduling care rotas, allocating care calls to staff, and maintaining accurate records of client interactions and care plans.
Key Responsibilities
The successful candidate will be responsible for:

• Allocating care calls to maximize staff efficiency
• Maintaining accurate records of client interactions and care plans
• Collaborating with the Senior Care Coordinator to update client files
• Providing exceptional customer service to clients and their families
Requirements and Qualifications
To succeed in this role, you will need:
- • A passion for delivering high-quality care services
• Excellent organizational and communication skills
• Ability to work independently and as part of a team
• Proficiency in Microsoft Office, particularly Word and Outlook
• Valid driver's license and access to a reliable vehicle
Estimated Salary
