About the RoleYou will be responsible for assessing client needs, developing personalized care plans, and implementing interventions that promote independence and community engagement. Your expertise as a Social Worker will be invaluable in this role, enabling you to build strong relationships with clients, carers, and other stakeholders.
Key Responsibilities- Care Planning: Develop and implement individualized care plans that meet the unique needs and goals of each client.
