Job Title: Nurse Practitioner - Rehabilitation (Nursing)

Job Summary:

Centstone is seeking a skilled Nurse Practitioner to join our team in a rehabilitation setting. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and developing individualized rehabilitation care plans.

Key Responsibilities:

  • Assessment and Monitoring: Conduct thorough assessments of patient conditions and progress, identifying areas for improvement and developing strategies for rehabilitation.
  • Care Plan Development: Collaborate with patients, families, and healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
  • Medication Administration: Administer medications and treatments as prescribed, ensuring accurate and timely delivery.
  • Patient Education: Provide patients and their families with education on rehabilitation exercises, self-care, and disease management.
  • Team Collaboration: Work collaboratively with physical therapists, occupational therapists, and other healthcare professionals to ensure comprehensive patient care.
  • Documentation: Accurately document patient care and progress, maintaining up-to-date records and reports.
  • Emotional Support: Provide emotional support and motivation to patients, helping them to achieve their rehabilitation goals.
  • Environment Safety: Ensure a safe and therapeutic environment for patients, adhering to infection control and safety protocols.

Requirements:

  • Education: Master's degree in Nursing or related field.
  • Experience: 2+ years of experience in rehabilitation nursing, with a focus on acute care and inpatient neuro rehabilitation.
  • Certifications: Current BLS certification and Epic experience preferred.
  • Skills: Strong assessment and monitoring skills, excellent communication and interpersonal skills, and ability to work effectively in a team environment.

  • Shift: 12-hour nights, every other weekend.
