Sub-Acute Care Opportunity at Gloucestershire Health and Care NHS Foundation Trust

We are seeking an experienced Occupational Therapist to join our team at Tewkesbury Community Hospital, Abbeyview Ward. The ideal candidate will be a qualified therapist with a passion for providing high-quality care in a sub-acute setting.

About the Role

The successful candidate will work closely with a multidisciplinary team to provide person-centred care to patients requiring rehabilitation and end-of-life care. This role involves assessing patients with complex needs, developing and implementing individualised care plans, and coordinating safe discharges.

Key Responsibilities

• Assessing patients' physical and cognitive abilities to identify areas for improvement
• Developing and implementing individualised care plans to achieve rehabilitation goals
• Collaborating with healthcare professionals to ensure seamless care coordination

Requirements

To be considered for this role, you must have:

• A current and valid registration with the Health Professions Council
• A Diploma or Degree in Occupational Therapy
• Experience working in a sub-acute care setting
