Job Title: Community Health Nurse

We are seeking a highly motivated and dynamic Community Health Nurse to join our team in Mid Bedfordshire. As a qualified Specialist Practitioner in District Nursing, you will be responsible for delivering high-quality nursing care to patients in their own homes.

Main Responsibilities:
  • Manage a caseload of patients and provide responsive, high-quality nursing care.
  • Work collaboratively with the team to achieve real improvements in patient outcomes.
  • Develop and implement effective care plans to meet the needs of patients.
  • Provide education and support to patients and carers to enable them to manage their conditions effectively.
  • Participate in the training and mentorship of students during placements.
Requirements:
  • Post-registration qualification in a relevant clinical field.
  • Relevant teaching/mentorship qualification and evidence of teaching skills.
  • Experience of managing a caseload and providing high-quality nursing care.
  • Ability to work collaboratively with the team and develop effective care plans.
  • Good oral communication skills and fluency in the English language.
