Job Summary:

We are seeking a skilled Pharmacy Technician/ Administrator to join our team at Forest Health Group. As a key member of our clinical pharmacy team, you will provide essential support to our pharmacists and clinicians, ensuring the safe and accurate supply of prescribed medications to our patients.

Key Responsibilities:
  1. Oversee the conversion of patients to Electronic Repeat Prescribing (ERD).
  2. Process Medication Safety Alerts, communicating any such notifications to patients as required.
  3. Regular liaison with community and acute hospital pharmacists.
  4. Support the achievement of the practices prescribing Quality and Outcomes Framework targets.
  5. Deal with medication queries, including acute medication requests, patient medication and prescription queries, with clinical support as required.
  6. Update and maintain accurate patient medication records on the practice clinical computer system, including advice given and action taken.
  7. Assist the practice with data analysis of prescribing behavior in line with NICE guidance and local clinical commissioning directives.
  8. Assist with the development and review of medicine audits.
  9. Provide a contact for patients and colleagues with medication queries.
  10. Project work/prescribing initiatives under the support of the clinical/prescribing staff.
  11. Link with hospital and community pharmacy colleagues to address discrepancies, supply issues and queries within competency.
  12. Liaise with the wider health care team, including primary care staff, community nurses, and care home staff, with medication queries.
  13. Undertake regular audits using different tools, for example, to identify priority patients for medication review.
  14. Document interventions made to patients' medications in their health care records to ensure there is a clear audit trail and changes are clearly accounted for.
  15. Attend local meetings seeking to improve access to medicines or repeat prescribing processes.
  16. Seek advice from the patients' GP or practice lead GP as appropriate.
